PSV medical examinations can be arranged at the surgery in Sheffield at the following times:
- Monday 18:00 and 18:30
- Tuesday 18:00 and 18:30
- Wednesday 18:00 and 18:30
- Thursday and Friday: not available
- £80, including the eye test

What to bring
- i.d. – Driving licence + proof of address eg utility bill
- Fresh urine specimen – ask for a pot at the reception desk
- Glasses or contact lenses that you may need for driving
- D4 form – please prefill your name and date of birth on the footer of each page

- There is a free public car park outside with 2 hour limit
- Lorry parking nearby on lay-by on the dual carriageway, 0.4 miles SW of the surgery
